1.       Please complete the RMA form with detailed descriptions of the problem with your item(s). Email this completed RMA request form to rma@ivitex.com

2.       The RMA Dept. will respond back to you a RMA number or provide a reason for RMA denial

3.       After your receive a RMA number, write it in the form field below. Make extra copies of your completed RMA form and original purchase ivitex invoice(s) include one of each of your copies with your carefully boxed returning item(s).

4.       Email one scanned copy of the original purchase ivitex invoice(s) showing the matching serial numbers of the returning item(s).

5.       After you receive a RMA number, then you may ship back your defective products to ivitex at the address indicated on the top of this form. Your assigned RMA number is valid for 5 business days from the date of issue. Please write the RMA number on the outside of the returning package (on the address line)
